4. Checklist of necessary documents:
   a) Completed Application Forms
   b) Original and photocopy of the child's Identification Documents (e.g., Birth Certificate, Passport, visa, etc.)
   c) Original and photocopy of the parent's/guardian's Identity Card, Passport or other Identity Documents
   d) One passport-size photo of the child
   e) Original and photocopy of the address proof (e.g. public housing tenancy agreement/ water/ electricity/ town gas/ residential telephone bills)

        * Bank statement is not accepted

   f) Original and photocopy of Birth Certificate of brothers and sisters studying in SEKPS, if any.
   g) Photocopy of the student handbook of brothers and sisters studying in SEKPS, if any.
